The movement you're showing is 3/0 size. The problem you describe is typical of wristwatches in 3/0 size (not just Illinois). You're probably used to smaller wristwatch movements in which there is a setting lever that engages a groove on the stem. The little bump on the lever holds the stem in place...

This movement would originally have been in a hunter-style pocket watch case. At some later date someone, probably a jeweler, transferred it to a generic wristwatch case as pocket watches fell out of style. I think that the Hamilton-Illinois catalog drawings do not always match the actual bracelets that showed up on the watches. The catalog image you show comes from p. 32 of the 1955 Hamilton catalog.
